- Official transcripts for all college work at an accredited US/Canadian institution
- 3 letters of recommendation: one from a science professor, one from a dentist with whom the applicant volunteered and one from an advisor, researcher or mentor.
- 1 letter of recommendation may be sent instead from a member of a pre-professional health advisory committee.
- Electronic letters preferred, paper letters also accepted but must be originals on letterhead
