- Degree: All applicants must earn a Bachelor's degree by the time of enrollment.
- All dental prerequisites must be completed in the U.S. or Canada

- General Chemistry with Lab (8 credits), or Inorganic Chemistry with Lab (8 credits)

- 3 letters from professors (at least two in sciences). 4th letter may be from supervisor, dentist, or other professional relation.

Interview
- Candidates selected for an interview will be notified by phone to select a date that is convenient for the applicant based on the HSDM available dates.
- Interviews are held on Mondays and Fridays starting late September until all selected applicants have been interviewed (generally by the end of February)
- One-on-One Interviews are conducted at the School of Dental Medicine by two members of the Admissions Committee.
- Students should plan to stay the day at the School since the day lasts from 8:30 – 2:00 (including lunch, tour and information sessions).
- Applicants must submit the following before their interview day:
- High school transcript (including SAT scores, if applicable)
- HSDM Supplemental Questionnaire
- 2x2 passport photo
